The program integrates seamlessly with iKnowMed to provide clinicians point-of-care tools for Innovent Oncology’s Integrated Cancer Solutions, including: Level I Pathways, Patient Support Services, and Advance Care Planning. The system also provides the detailed clinical documentation required for providers to fully benefit from the program.
iKnowMed fully supports Innovent Oncology’s Level I Pathways, which are designed by oncologists to deliver the highest quality, evidence-based cancer care to patients. For practices using the iKnowMed application while actively participating in Innovent Oncology in their market, recommended Level I Pathways are included among the diagnosis-specific regimen options presented after staging and diagnosis are selected for a patient in the iKnowMed application. However, if the oncologist feels that the recommended Level I Pathways are not appropriate for the patient, they can always choose an alternative treatment.
Through Innovent Oncology, patients can be enrolled in Patient Support Services, a disease management program directed by a practice physician and designed to ensure patient understanding of their disease and compliance with their treatment plan. Working closely with practice staff, oncology-certified registered nurses provide patients focused education specific to their disease using evidence-based, standardized education materials. Patients receive regular outreach support tailored to their needs including specific drugs, their potential side effects and how to manage them should they arise.
Innovent Oncology Advance Care Planning is designed to facilitate timely, sensitive discussions between the oncologist and their staff with patients and their loved ones as they confront transition from active/curative treatment to palliative care. The same cancer care team that guides patients through their fight to overcome cancer is also able to introduce end-of-life concepts to patients, including treatment options, Advance Directives, and palliative care and hospice. These discussions empower patients, increase their satisfaction with care, and give everyone peace of mind.
